Chronic ObstructivernPulmonary Disease (COPD) imposes a significant burden, with exertional dyspnoearnand desaturation driven by dynamic hyperinflation. Positive expiratory pressurern(PEP) techniques, such as pursed lip breathing, alleviate these symptoms butrnare inconsistent in practice. The PEP Buddy, a novel hands-free oral PEPrndevice, delivers 4-10 cm H₂O of expiratory pressure, offering a practicalrnalternative for COPD management. This review synthesizes evidence on the PEPrnBuddy’s design, mechanisms, clinical benefits, and integration into pulmonaryrnrehabilitation. Studies indicate it improves dyspnoea in 71.8% of COPD patientsrnand mitigates desaturation in 35.7% of those affected, enhancing quality ofrnlife (QoL) and exercise tolerance. Its hands-free design supportsrnself-management, complementing interventions like breathing exercises andrnphysical activity. Limitations include minor usability issues (e.g., salivarnbuildup) and limited long-term data. The PEP Buddy holds promise as an adjunctrnto comprehensive COPD care, warranting further research on optimal use andrnscalability.
